Intro
Echinocin is a non-competitive inhibitor of β-1, 3-D-glucan flatase. β-1.3-D-dextran synthesis enzyme responsible for the biosynthesis of the main structural component of fungal cell wall β-1, 3-d-dextran. The U.S. Food and Drug Administration (FDA) has approved Echinocinins for the treatment of esophageal and invasive candidiasis, candidemia, and empiric therapy in patients with febrile neutropenia, and prophylactic therapy in patients receiving hematopoietic stem cell transfer (HSCT). The Infectious Diseases Society of America (IDSA) recommends echinocalcin in most patients with neutropenic candidemia, and in non-neutropenic patients, echinocalcin in place of azole drugs, and echinocalcin is the preferred drug for patients with moderate to severe disease and those who are resistant to azole drugs.
Peculiarity
Echinocins have a wide range of activities against most common Candida species, showing fungicidal activity in vitro. In contrast, they have an inhibitory effect on susceptible molds such as Aspergillus, which can lyse and enlarge the tip of mycelia, change the morphology of mycelia, and the organization of cell walls. However, the inhibitory spectrum of Echinocins is limited, and they are not active against Mucor, Cryptococcus and Fusarium. Echinocins are highly active against triazole-resistant candida glabrata and C. krusei, as well as some Candida biofilms, because their mechanisms of action are not related to azole and are not substrates for the multidrug effluents found in highly azole-resistant strains.
